Bradford Breathing Better (BBB)

  • Improving access and increase uptake to pulmonary rehabilitation and developing a local Community Partnership foot print delivery. In addition, we are also supporting improvements across the whole of West Yorkshire and Harrogate (WY&H)
  • Admissions avoidance scheme – With a focus on high risk patients: medication reviews, intensive care planning / personalised care and including housebound and people in care homes
  • Spirometry – Looking at development of hub model, reduction in inappropriate diagnosis, improve care longer term, skill up work force and streamline spirometry service model
  • Self-care – Includes support for patients to understand medication (concordance) and their condition, exacerbations and management and engaging with local Breathe Easy Groups.

Healthy Hearts

  • Bradford’s Healthy Hearts is now business as usual
  • Bradford district and Craven delivery of regional West Yorkshire & Harrogate programme which relates to improvements related to cholesterol, Hypertension and Atrial Fibrillation (outcomes and timescales agreed)
  • Developing a local review clinic for patients with severe heart failure to improve care and management and reduce repeated hospital admissions
