WHAT WE DO

DIABETES

Diabetes is one of Bradford’s key healthcare priorities. The work is focusing on the following two areas to improve individual’s health Primary prevention – reducing the risk of people developing diabetes Diabetes care – once diagnosed, supporting people to manage their condition and reduce the risk of future complications. OUT OF HOSPITAL

This work covers all services that are provided in a community setting that supports individuals’ physical, mental health and care needs, so that people can live as independently as possible, with support, at home.  If individuals need hospital care, the focus will be on supporting people to get well and regain their independence so they can return home.
